Who we can support
Adults who need support to live safely and independently at home or in supported living settings — including older people, adults with physical disabilities, people living with dementia, and people recovering after a hospital stay. See our care services for the full scope.
Geographic area
We provide care across Somerset, based in Taunton. See Areas We Cover for detail, or include the client's postcode in your referral enquiry and we will confirm coverage directly.
What we need to know
- The broad level of support required and the requested service
- The client’s area or postcode (coverage check)
- Required start timeframe
- Funding or referral route where known
- Whether you would prefer a callback instead of written detail
Our response process
- Acknowledgement. Referral enquiries are picked up by our referrals contact and acknowledged quickly.
- Capability & coverage check. We confirm honestly whether we can meet the support level in that area, and come back with any questions.
- Assessment & confirmation. Where we accept, we arrange the assessment and confirm the plan, start date and contact route in writing.
If we cannot accept a referral, we will say so promptly and clearly rather than leaving it open.
