Older people and adults who need extra help with everyday life but want to remain in the home they know — whether that is help after a change in circumstances, ongoing support with daily routines, or a combination of both.

How this support is personalised

Care is planned around the individual — their routines, preferences, abilities and the things that matter to them. Visit times, the tasks covered and who provides the support are agreed with you before care begins, and reviewed as needs change.

What families can expect

You can expect a written care plan, consistent carers wherever possible, records kept for every visit and a named contact for families. If anything changes, we review the plan with you rather than simply carrying on.
