How this support is personalised
Support does things with people, not for them. Hours and staffing are aligned to the person's support plan, and the emphasis is always on building independence at the person's own pace.
What families can expect
CDD provides the care and support within a supported living arrangement — we do not provide the housing itself. Your written support plan sets out exactly what is included, and it is reviewed with you regularly.
How the service starts
Every support plan begins with a conversation and an assessment. We listen first, agree the plan with you before care begins, and review regularly so support keeps fitting the person. See how arranging care works.
Available across Somerset — see Areas We Cover for detail.
